Cardiac rehabilitation in the paediatric Fontan population: development of a home-based high-intensity interval training programme

Introduction:
We evaluated the safety and feasibility of high-intensity interval training via a novel telemedicine ergometer (MedBIKE™) in children with Fontan physiology.
Methods:The MedBIKE™ is a custom telemedicine ergometer, incorporating a video game platform and live feed of patient video/audio, electrocardiography, pulse oximetry, and power output, for remote medical supervision and modulation of work. There were three study phases: (I) exercise workload comparison between the MedBIKE™ and a standard cardiopulmonary exercise ergometer in 10 healthy adults. (II) In-hospital safety, feasibility, and user experience (via questionnaire) assessment of a MedBIKE™ high-intensity interval training protocol in children with Fontan physiology. (III) Eight-week home-based high-intensity interval trial programme in two participants with Fontan physiology.
Results:There was good agreement in oxygen consumption during graded exercise at matched work rates between the cardiopulmonary exercise ergometer and MedBIKE™ (1.1 ± 0.5 L/minute versus 1.1 ± 0.5 L/minute, p = 0.44). Ten youth with Fontan physiology (11.5 ± 1.8 years old) completed a MedBIKE™ high-intensity interval training session with no adverse events. The participants found the MedBIKE™ to be enjoyable and easy to navigate. In two participants, the 8-week home-based protocol was tolerated well with completion of 23/24 (96%) and 24/24 (100%) of sessions, respectively, and no adverse events across the 47 sessions in total.
Conclusion:The MedBIKE™ resulted in similar physiological responses as compared to a cardiopulmonary exercise test ergometer and the high-intensity interval training protocol was safe, feasible, and enjoyable in youth with Fontan physiology. A randomised-controlled trial of a home-based high-intensity interval training exercise intervention using the MedBIKE™ will next be undertaken.

Background
Depression and obesity are highly prevalent, and major impacts on public health frequently co-occur. Recently, we reported that having depression moderates the effect of the FTO gene, suggesting its implication in the association between depression and obesity.
AimsTo confirm these findings by investigating the FTO polymorphism rs9939609 in new cohorts, and subsequently in a meta-analysis.
MethodThe sample consists of 6902 individuals with depression and 6799 controls from three replication cohorts and two original discovery cohorts. Linear regression models were performed to test for association between rs9939609 and body mass index (BMI), and for the interaction between rs9939609 and depression status for an effect on BMI. Fixed and random effects meta-analyses were performed using METASOFT.
ResultsIn the replication cohorts, we observed a significant interaction between FTO, BMI and depression with fixed effects meta-analysis (β=0.12, P = 2.7 × 10−4) and with the Han/Eskin random effects method (P = 1.4 × 10−7) but not with traditional random effects (β = 0.1, P = 0.35). When combined with the discovery cohorts, random effects meta-analysis also supports the interaction (β = 0.12, P = 0.027) being highly significant based on the Han/Eskin model (P = 6.9 × 10−8). On average, carriers of the risk allele who have depression have a 2.2% higher BMI for each risk allele, over and above the main effect of FTO.
ConclusionsThis meta-analysis provides additional support for a significant interaction between FTO, depression and BMI, indicating that depression increases the effect of FTO on BMI. The findings provide a useful starting point in understanding the biological mechanism involved in the association between obesity and depression.

Early-time periodic injection and extraction in an inclined confined aquifer

We consider the periodic injection and extraction of fluid from a horizontal line well in an inclined saturated aquifer of finite thickness, $H$, as part of an aquifer thermal energy storage system. We focus on the case in which the injected fluid is dense relative to the original fluid in the aquifer and hence tends to run off downslope, so that during the extraction cycle a mixture of original aquifer fluid and injected fluid is produced. We study the controls on the composition of the extracted fluid, distinguishing between the cases where there is an open boundary downslope or upslope of the source, with the other boundary being sealed. We find that initially the fraction of original aquifer fluid recovered during an extracted cycle decreases from cycle to cycle, and at long times this fraction asymptotes to a value, $\mathscr{F}>0$, that depends on ${\it\Gamma}=H(S\cos ({\it\theta}){\it\phi}^{2}/{\it\tau}Q^{2})^{1/3}$ and ${\it\Pi}=HS\sin ({\it\theta}){\it\phi}/Q$, where ${\it\tau}$ is the time period of injection, ${\it\phi}$ is the porosity of the aquifer, $Q$ is the injection rate, $S$ is the speed of the buoyancy driven flow and ${\it\theta}$ is the angle of inclination to the horizontal. At long times the flow asymptotes to a quasi-steady exchange flow solution that develops in an inclined saturated porous layer of thickness $H$ in contact with a stratified fluid reservoir, filled to thickness $h_{0}({\it\Gamma},{\it\Pi})<H$ with relatively dense fluid. This enables us to calculate the average composition of the extracted fluid, which is of concern if the original aquifer fluid contains contaminants. Since the produced aquifer fluid is driven by this quasi-steady exchange flow, we show that contaminants downslope of the well will be produced after a finite time, whereas contaminants upslope of the well typically remain in the aquifer, irrespective of the far-field boundary conditions. We successfully test the models with a series of analogue experiments of both the injection extraction flow and the exchange flow using a Hele-Shaw cell.

Studies of trade routes across Southeast Asia in prehistory have hitherto focused largely on archaeological evidence from Mainland Southeast Asia, particularly the Thai Peninsula and Vietnam. The role of Indonesia and Island Southeast Asia in these networks has been poorly understood, owing to the paucity of evidence from this region. Recent research has begun to fill this void. New excavations at Sembiran and Pacung on the northern coast of Bali have produced new, direct AMS dates from burials, and analytical data from cultural materials including pottery, glass, bronze, gold andsemi-precious stone, as well as evidence of local bronze-casting. This suggests strong links with the Indian subcontinent and Mainland Southeast Asia from the late first millennium BC, some 200 years earlier than previously thought.

We consider the periodic injection and extraction of fluid from a line well in a horizontal saturated aquifer of finite thickness as part of an aquifer thermal energy storage system. We focus on the case in which the injected fluid is dense relative to the original fluid in the aquifer and we explore the competition between the driving pressure and buoyancy force in controlling the dispersal of the injected fluid through the aquifer. We show that, after each cycle, a progressively larger fraction of the injected fluid is extracted, while the remainder of the injected fluid gradually migrates away from the well such that, after time $\def \xmlpi #1{}\def \mathsfbi #1{\boldsymbol {\mathsf {#1}}}\let \le =\leqslant \let \leq =\leqslant \let \ge =\geqslant \let \geq =\geqslant \def \Pr {\mathit {Pr}}\def \Fr {\mathit {Fr}}\def \Rey {\mathit {Re}}t$, the position of the leading edge of the injected fluid, $x_{nose}(t)$, scales as $ x_{nose}(t)\sim x_{nose}(\tau )\sqrt{t/\tau }$, where $\tau $ is the period of injection. If the fluid is extracted from the base of the layer, then, near the well, the thickness of the injected fluid at the end of the extraction cycle tends to a constant value, which decreases with injection rate. We also show that there is a class of self-similar exchange-flow solutions that develop when a saturated porous layer of thickness $H$ is in contact with a stratified fluid reservoir, filled to thickness $F_0H<H$ with relatively dense fluid, and with original reservoir fluid above this level. We show that these solutions coincide exactly with the far-field flow produced by the injection–extraction cycles. We successfully test the models with a series of analogue experiments of both the injection–extraction flow and the exchange flow using a Hele-Shaw cell. In the case that the fluid is injected and extracted from the top of the aquifer, the value $F_0$ tends to unity in all cases, although the convergence time depends on the rate and period of injection, the buoyancy speed and the vertical extent and the porosity of the aquifer. We use the model to explore how the concentration of reservoir fluid in the produced fluid varies as the system evolves from cycle to cycle, and we also examine the time required to transport a localised but distant contaminant to the production well through the far-field exchange flow. Finally, we consider the analogous axisymmetric injection–extraction flow problem, and show, through both numerical solution of the governing equations and experiment, that, although there is no simple class of similarity solutions, the fraction of injected fluid that is extracted progressively increases in each cycle.

INTRODUCTION
One of the most striking features of “continental” philosophy, and especially of the theories arising from German-speaking philosophy in the late eighteenth and most of the nineteenth century, consists in the fact that the concept of a ‘system’ plays a prominent role as a criterion for evaluating them. In almost no other age or context have the ideas of systematicity, systematic wholeness, and the form of a system been so bound up with the concept of philosophy itself. And this is true not only regarding positive evaluations but also negative ones. For some philosophers in this time the fulfillment of the criterion of systematicity was a direct and necessary condition for redeeming the claim to have provided a philosophical theory about something. Others, however, have seen the form of a system as an obstacle to any philosophical theory’s offering us any illuminating results whatever regarding any object at all.
That such controversial assessments could have arisen at all shows clearly all by itself that in this period there was a determinate spectrum of expectations regarding resorting to the concept of a system. This spectrum cannot be adequately accounted for if one is content to assume that the concept of a system, as in the eighteenth century Wolffian tradition, stands only for the idea of a certain methodical ordering of sentences (judgments, propositions) – thus for something like “being a science” (Wissenschaftlichkeit). In the late eighteenth and nineteenth centuries, the idea of a science was, to be sure, bound up with that of a system but did not exhaust it. We can see this if we attend to the fact that on the basis of a description of the interest in systematicity that relies on the concept of being scientific we cannot make sense of the fact that a conception of philosophy that does without the claim to systematicity does not automatically give up the claim to being scientific; nor on this basis can we understand how in this period the suspicion of systematicity could have arisen at all. In other words, we cannot in this way understand how a philosophy that is “only” scientific can be presented as a serious philosophical undertaking without making any claim to be systematic.

We examine the oscillatory motion of fluid which spreads under gravity along a horizontal impermeable boundary through a porous medium controlled by the periodic injection and extraction of fluid from a horizontal well. Over the first few cycles the volume of injected fluid exceeds that which is extracted owing to the gravitational spreading of the current. However, after many cycles, these volumes converge and the flow develops into two regions. Near the source there is a zone in which the depth of the fluid varies periodically with each cycle, where is the fluid injection rate, is the injection or extraction time, is the speed of the buoyancy-driven flow and is the porosity. The current attains its maximum depth, at the source, where the minimum depth equals zero. At long times, the current depth at is approximately constant, , and beyond this point, the current spreads horizontally, driven by an effective flux , so that the length of the current increases as . We confirm these predictions with new experiments using a Hele-Shaw cell. We also model the evolution of the thermal front which develops if the injected fluid is hotter than the formation temperature. We find conditions under which all the extracted fluid is hot but owing to the mismatch between the volume of injected and extracted fluids, not all the injected thermal energy is recovered, and the surrounding rock heats up.

An overview is given of the development of advanced nanoporous carbons as storage ma-terials for natural gas (methane) and molecular hydrogen in on-board fuel tanks for next-generation clean automobiles. The carbons are produced in a multi-step process from corncob, have surface areas of up to 3500 m2/g, porosities of up to 0.8, and reversibly store, by physisorp-tion, record amounts of methane and hydrogen. Current best gravimetric and volumetric storage capacities are: 250 g CH4/kg carbon and 130 g CH4/liter carbon (199 V/V) at 35 bar and 293 K; and 80 g H2/kg carbon and 47 g H2/liter carbon at 47 bar and 77 K. This is the first time the DOE methane storage target of 180 V/V at 35 bar and ambient temperature has been reached and exceeded. The hydrogen values compare favorably with the 2010 DOE gravimetric and volu-metric targets for hydrogen. A prototype adsorbed natural gas (ANG) tank, loaded with carbon monoliths produced accordingly and currently undergoing a road test in Kansas City, is de-scribed. A preliminary analysis of the surface and pore structure is given that may shed light on the mechanisms leading to the extraordinary storage capacities of these materials. The analysis includes pore-size distributions from nitrogen adsorption isotherms; spatial organization of pores across the entire solid from small-angle x-ray scattering (SAXS); pore entrances from scanning electron microscopy (SEM) and transmission electron microscopy (TEM); H2 binding energies from temperature-programmed desorption (TPD); and analysis of surface defects from Raman spectra. For future materials, expected to have higher H2 binding energies via appropriate sur-face functionalization, preliminary projections of H2 storage capacities based on molecular dy-namics simulations of adsorption of H2 on graphite, are reported.
